Should You Choose Testosterone Cream or Injections?

One of the first questions I hear from patients is: "Should I do testosterone injections or testosterone cream?" The honest answer? Neither is inherently better. There isn't a "best" form of testosterone replacement therapy (TRT) — only the option that's best for you.

I've seen patients thrive on injections. I've seen others feel incredible using compounded testosterone cream. The right choice depends on your lifestyle, your goals, how your body metabolizes testosterone, and what you're hoping to achieve from treatment. First, What Is Testosterone Replacement Therapy?

TRT is designed to restore testosterone levels in men who have both symptoms of testosterone deficiency and laboratory evidence of low testosterone. When testosterone levels decline, men commonly notice fatigue or low energy, reduced libido, erectile dysfunction, difficulty building or maintaining muscle, increased body fat, brain fog or poor concentration, loss of motivation, slower recovery after exercise, and mood changes or decreased confidence.

Testosterone Injections

Testosterone Cream

Compounded testosterone cream has become increasingly popular — and for good reason. Unlike traditional testosterone gels, compounded creams are often applied to the scrotal skin, where absorption is significantly greater. This allows many men to achieve excellent testosterone levels with relatively small doses.

One of the unique advantages of testosterone cream is that it tends to produce higher levels of dihydrotestosterone (DHT). Healthy DHT levels contribute to libido, erectile quality, sexual function, confidence, motivation, and energy. In fact, some men report feeling their absolute best on testosterone cream, likely because of its effect on DHT production.

So…Which One Works Better?

I've seen incredible results with both. Some patients feel fantastic on injections. Others wouldn't switch away from cream if you paid them. The best testosterone protocol isn't determined by what's most popular on social media — it's determined by what helps you feel your best while keeping your hormones safe and well monitored.

Testosterone injections may be a great fit if you prefer less frequent dosing, want predictable absorption, are comfortable with self-injections, or are looking for a more affordable option. Testosterone cream may be a better choice if you prefer to avoid needles, like a simple daily routine, or are interested in the potential benefits of higher DHT production.
